Housing Stability Program - Landlord Opt-In

LANDLORD OPT-IN OVERVIEW

Thank you for your interest in participating in CHN's Housing Stability Program! We are actively seeking landlords and property managers who may be interested in working with our Housing Stability team to connect prospective tenants to available units in the city of Cleveland.

Please fill out the following form if you are interested in being a part of CHN's outreach list to assist us with connecting our Housing Navigation clients to your available units.
PROGRAM OVERVIEW

Through CHN's Housing Navigation program, eligible households may receive up to $5,000 in rental assistance towards their security deposit (equivalent to one month's rent) and first three (3) months rent. 

To be eligible for the program:
  1. Tenant:
    1. Must be at or below 80% Area Median Income
    2. Must participate in Rental Housing/Eviction Prevention Counseling with a HUD-certified Housing Counselor
    3. Must be able to maintain their housing payments after CHN's rental assistance ends
  2. Unit/Property:
    1. Must be located in the city of Cleveland
    2. Must be registered with the City of Cleveland rental registry 
    3. Must be in compliance with the City of Cleveland's lead safe ordinance
    4. Must not have any unaddressed tax or mortgage delinquencies
    5. Must not be at-risk of/in foreclosure
    6. NOTE: Through CHN's Housing Navigation program, we cannot assist with and/or provide funding for units located outside of the city of Cleveland.
  3. Landlord:
    1. Must provide an executed, written lease
    2. Must provide a W-9
    3. Must complete a Landlord Resolution Plan
    4. Provide relevant rental registry and lead safe certification documentation
    5. May not live in the same unit as the tenant/applicant
